A great way to understand the various health plan programs being used to improve patient safety is AHIP’s latest Innovations Report entitled “Innovations in Reducing Preventable Hospital Admissions, Readmissiones and Emergency Room Use“. This report, conducted by AHIP’s Center for Research and Policy highlights “a wide range of patient-centered initiatives developed and refined over the last several decades” that have been moving the delivery system toward higher quality and improved patient safety.
The entire report is a great compendium of specific examples of programs, tools and initiatives being employed by health plans today to improve patient safety and quality. The report has five chapters:
-
Breaking New Ground with Innovative Care and Payment Models
-
Helping Patients Transition from Hospital to Home
-
Improving the Quality of Life for Patients at High Risk
-
Caring for Frail Patients at Home
-
Helping Reduce Preventable Use of Emergency Rooms
The programs and tools highlighted in this latest report are exactly the types of reforms today’s new initiative is meant to apply across the entire system – public and private. Health plans stand ready to work with all stakeholders on this initiative.
